◆免疫はウイルスとどう戦うか？
＊粘膜免疫
分泌型IgA
※ 気管関連リンパ組織（bronchus-associated lymphoid tissue: BALT）
気管には誘導組織としてBALT
＊肺胞のマクロファージ
※肺胞にはマクロファージがいて、
自然免疫として入ってきたウイルスを貪食する
＊獲得免疫
細胞障害性T細胞（キラーT細胞）
*特定なウイルスによる感染細胞を破壊する細胞障害性T細胞（獲得免疫）
ウイルスを貪食したマクロファージは
表面にMHCクラスⅠで抗原提示
↓
抗原提示細胞がリンパ節でTリンパ球を活性化
ウイルスの抗原に対する特異性あるTCRを持った
Tリンパ球（エフェクター細胞）が分裂増殖
↓
増加したTリンパ球はリンパ節から出て、胸管を経由して
全身の血管へ流れていく
サイトカインによって
リンパ球が集まる
↓
ウイルスが感染した肺胞上皮細胞などをみつけると
細胞傷害性T細胞はそれを攻撃する。
細胞傷害性T細胞から放出された
パーフォリンが細胞に孔をあけ、
爆弾のようなグランザイムが細胞内に投げ入れられると
細胞はアポトーシスをおこして破壊され消失する。
＊血管内のIgG
ヘルパーT細胞がB細胞を形質細胞に分化させ、
形質細胞がIgGなどの抗体を産生する。
※ウイルスは細胞内に感染する病原体なので
細胞の外の体液性免疫である血管内のIgG抗体は
メインでなく補助的な免疫である。
◆死因はARDSか血管の閉塞
＊
※血管がつまる理由は２種類
＃炎症（サイトカインストーム）
炎症により血中の
サイトカインが亢進して
血管がつまりやすくなる。
＃血管にウイルスが入って血管内皮細胞に感染
＊人体の組織が正常な間は、
血管の中にウイルスは侵入できない。
２～３週間かけて悪化して
人体の正常構造が壊れると、
血管内にウイルスが入ってしまうことがあるが
そうなることは少ない。
血管の中にウイルスが入ると、
血管の内皮細胞に感染して
血管閉塞する。
◆１つのウイルスに対する免疫が弱いから
ウイルスが１億以上に増殖
＃免疫が弱いのでウイルスが増殖してサイトカインストーム
ウイルス１つに対する免疫の
反応は弱い。
１億以上にウイルスが増殖すると
全体として免疫は亢進
＃ウイルスの増殖を止めれば、免疫は暴走しない
悪化前にアビガンやイベルメクチン
でウイルスの増殖を
止めることが大切。
※ステロイドをCOVID-19の
初期無症状期に使うと、
ウイルスへの免疫が抑制されて
かえってウイルスが増えます。
発症後の炎症には
肺胞上皮細胞への感染なので、
吸入のステロイドの
オルベスコが有効。
血液への#デキサメタゾン
は末期、死亡回避用の薬です。
