うつした人もうつされた人も発症するまで
時限爆弾に気がつかない
◆コロナウイルスはACE２に結合し細胞に侵入して増殖
新型コロナウイルスは人体の細胞表面のACE２に結合し
そのRNAが細胞内に入る。細胞の中でウイルスの複製が
行われ、ウイルスは増殖する。
◆新型コロナウイルスは免疫を抑制する
細胞にウイルスが感染すると、免疫が正常であれば
細胞内にサイトカインやインターフェロンが産生される。
マクロファージからもサイトカインがでて発熱する。
そして感染した細胞をリンパ球が破壊するので
痛みを感じる。
ところが新型コロナウイルスはRNAとなって細胞内に侵入すると
サイトカインやインターフェロンの産生を抑制してしまう。
免疫細胞を呼ぶサイトカインなどが放出されないと、
感染場所にリンパ球（細胞障害性Ｔ細胞）は集まらない。
免疫を担当する細胞が攻撃しないので、症状がでない。
◆最初はマクロファージなどによる自然免疫
マクロファージがウイルスを貪食
↓
マクロファージからインターロイキンなどの
サイトカイン
↓
サイトカインが脳の血液脳関門の手前まで流れる
↓
サイトカインによってプロスタグランジンE2
↓
プロスタグランジンE2が視床下部の
体温調節中枢に作用すると
発熱
◆時限爆弾を止めないと爆発してARDS
発症者をみつけ、それが時限爆弾のような
新型コロナウイルス感染症だったら、
そのカウントダウンを止めないと
ARDSに進行してしまう
COVID-19時限爆弾のカウントダウン
[028] 集団の1人が感染するが無症状
[021] 集団の複数の人に感染がひろがる
[014] 症状がないので、感染した自覚なく普通に生活
[007] 7日前に発熱、倦怠感などの初発症状が出現 ARDSの一週間前
※回復しない場合は、発症から約一週間でARDS
◆時限爆弾のカウントダウンを止められる薬

※イベルメクチンもウイルスの増殖を抑制できる
※フサンはウイルスが細胞内に侵入するのを
抑制するので、時限爆弾が入った細胞が増えない。
◆特定なウイルスによる感染細胞を破壊する細胞障害性T細胞（獲得免疫）
ウイルスを貪食したマクロファージは
表面にMHCクラスⅠで抗原提示
↓
抗原提示細胞がリンパ節でTリンパ球を活性化
ウイルスの抗原に対する特異性あるTCRを持った
Tリンパ球（エフェクター細胞）が分裂増殖
↓
増加したTリンパ球はリンパ節から出て、胸管を経由して
全身の血管へ流れていく
サイトカインによって
リンパ球が集まる
↓
ウイルスが感染した肺胞上皮細胞などをみつけると
細胞傷害性T細胞はそれを攻撃する。
細胞傷害性T細胞から放出された
パーフォリンが細胞に孔をあけ、
爆弾のようなグランザイムが細胞内に投げ入れられると
細胞はアポトーシスをおこして破壊され消失する。
◆時限爆弾が爆発してARDS(急性呼吸窮迫症候群）
細胞障害性Ｔ細胞が新型コロナウイルスに感染した
肺胞上皮細胞を攻撃。
肺胞上皮細胞からサーファクタントがでなくなり、肺胞は虚脱。
血管透過性が亢進して血管からの滲出が肺胞の周囲にたまる。
肺胞から血管へ酸素が拡散しなくなって呼吸困難。
