断食vs間食！正しいのはどっち？NHKの「美と若さの新常識」対「ガッテン」
●断食と間食、正しい指導はどっち？
2017年6月1日のNHKの番組「美と若さの新常識」は、
断食がなぜ体によいかという話でした。
2018年12月12日のNHKガッテンは、
間食をすすめる内容でした。
学校に2人の先生がいて、
授業でそれぞれが逆の指導をしたら、、
生徒は混乱してしまいますね。
腹が減ったら、おやつを食べた方がよいのか、
空腹をがまんして、絶食すべきなのか、
どちらが正しいのでしょう？
NHKを見て、断食と間食のどちらが体に良いのかわからなくなった患者さんが、
多摩境内科クリニックに来院し、福富院長に質問しました。（再現ドラマ）
食事指導する時には、その前に健康診断が必要です。
あなたは太っているのか、痩せているのか。
糖尿病で薬を飲んだり、インスリンを注射したりしているのか。
症状はないけれども、24時間血糖を測定すると、
低血糖になっているのか。
まず個別に体の状態を調べ、その後でなければ適切な食事指導はできません。
低血糖がおきている人には間食が必要です。
そうでない健康な人は、絶食しても低血糖にはなりません。
糖尿病で内服薬を飲んだり、インスリンを注射している人は、
厳格に血糖値を下げようとするほど、低血糖になる
リスクが高まります。
重篤な低血糖をおこしてしまうと、血管の内皮が障害されて、
心臓の病気をおこしやすくなってしまうのです。
糖尿病でなく、低血糖症状を自覚しない人でも、24時間血糖を測定できる装置で
検査をすると、低血糖をおこしていることがあります。
低血糖をおこしていない、肥満者の場合は、
断食すると体の状態が改善します。
食事を中断しても肝臓などから補給されるので
血液の中の血糖値は下がらないのです。
「腹が減った」と「低血糖症状」は違います。
低血糖の時は、動悸、発汗、意識障害などの症状がでます。
食欲があったら、低血糖なのではありません。
食欲と低血糖症状を混同してはいけないのです。
間食と断食、どちらが正しいかは、
誰にそれを指導するかで決まります。
全員に１つの同じ指導ではなく、健康診断後の個別の栄養指導が必要なのです。
