お久しぶりです。総合内科専門医の福富です。
私は2016年8月に多摩境内科クリニックを開業しました。
そして現在も院長として働いています。
私は1999年から、感染症時の血液の中のリンパ球の絶対数を病院の外来で調べ始めました。
インフルエンザなどの急性期におけるリンパ球の変化を調べてみたのです。
リンパ球数は、風邪でも減りますが、インフルエンザだともっと激減します。
インフルエンザかどうかを判断するためには、
リンパ球の絶対数が1000/μL以下になったかどうかを見るといいです。
末梢血液の白血球数に、血液像の分画のパーセントをかけると、
リンパ球数の絶対数がわかります。
インフルエンザのピーク時には
600/μL以下にまでリンパ球が少なくなっていました。
このリンパ球の減少の程度をみることで風邪がインフルエンザかを診断できます。
これはインフルエンザ急性期における最も早い検査方法だと思います。
他の重症のウイルス感染症でも、リンパ球は著減します。
ノロウイルス感染症の時も、インフルエンザ感染症と同じような
リンパ球の著減現象がみられます。
南東北グループの渡邉一夫理事長によって2012年の8月に、新百合ヶ丘総合病院が開院しました。
開院準備期間から、私はその新百合ヶ丘総合病院の総合内科の立ち上げを行いました。
病院開院後の初回の冬に、
インフルエンザなどの感染症外来を毎日担当したのは私です。
病院周辺のありとあらゆる感染症患者さんが、
外来にやってきました。その時にも上記の方法の正しさを
確認し、電子カルテに記録を残していきました。現在も記録が残っているはずです。
2012年11月に新百合ヶ丘総合病院内で、
医師が集まった時に、この方法と、その結果について話しました。
覚えている先生も、まだいらっしゃるかもしれません。
